《健康状况声明书》的通知

自2020年11月14日起，自伊朗出发搭乘航班前往中国的外籍乘客，可在线申领《健康状况声明书》。外籍乘客可通过电脑或手机等终端登录网站：https://hrhk.cs.mfa.gov.cn/H5/或使用手机扫描下列二维码（见附图），进入登录界面，在线填写信息、申报健康状况、上传检测证明等材料。中国驻伊朗大使馆审核通过后，将通过该网站核发《健康状况声明书》（电子版）。《健康状况声明书》（电子版）以带“HDC”标识的二维码（见附件一）形式展示，外籍乘客登机时须出示电子版或纸质打印版，供航空公司核对。

Notice on Requiring China-bound Foreign Passengers to Obtain Health Declaration Code(HDC) Before Boarding from Iran

Starting from 14 Nov, 2020, all foreign passengers who are to fly from Iran to China can obtain the Health Declaration Code(HDC) online. Applicants can login to the website:https://hrhk.cs.mfa.gov.cn/H5/ via computer or cellphone or scan the QR code by cellphone (see picture below), fill in all the information required and upload the photos of COVID-19 PCR&IgM anti-body tests negative certificate. After being reviewed and proved by the Chinese Embassy in Iran, the passenger can obtain the Health Declaration QR Code with the "HDC" mark(see attachment I). When boarding the plane, foreign passengers must present the QR code on cellphone or a print for the airline to check.
